Provisional Membership

Required Documents

All applicants for Provisional membership are required to show evidence of:
CCPT QUALIFICATION
Either:- CPW Cert CCPT Foundational (Level 1) CertificateOr:- CPW Cert CCPT Advanced (Level 2) Certificate Or:- CPW PGrad Cert CCPT Certificate Or:If you hold another CCPT Qualification:- CCPT Qualification Certificate - 30 CCPT Supervision hours and personal endorsement from your CCPT supervisor - 160 hours CCPT training including training transcript- 100 hours of clinical CCPT work with clients. Include a log of your hours- 15 hours of personal therapy. Please include a log of hours, dates and signed by your therapist

If you have changed your name
Evidence of name change if you have changed your name and it differs from your name on your Certificates.
Before filling in the application form, please have all relevant documentation ready to upload prior to commencing your application. These documents do not need to be certified copies, however certificates, academic transcripts, statutory declarations, and/or signed letters from supervisors will be required.
